Medications while at school

  • All medications must be brought to the health clinic in original bottle with student's name affixed.
  • The school does not provide oral medications unless a parent speaks directly to the school nurse and gives verbal permission, and it is up to the nurses's discretion if medication is given.
  • The parent must assure the medication arrives safely to school.
  •  Parents must provide an authorization form or written note including the following:
    o Students Name
    o Medication Name, purpose of medication
    o Dosage of Medication and times to be given
    o Parent/Guardian Signature
  •  Medications must be FDA approved. No expired medications will be administered at any time.
