Volunteer/Chaperone/Guest Speaker Background Check

Thank you for your interest in serving as a volunteer, chaperone or guest speaker for our students at Frankfort High School. In order to serve in this capacity, we ask that you complete this form and agree to take part in a limited criminal history background check.
